Appeal Media Loan Late Fines

Do you feel you have been unfairly charged? Do you have mitigating circumstances you wish us to consider? Fill out this Appeal Form and tell us why your fines should be reduced or waived.

Appeal process instructions:

  • Appeals must be complete to be accepted.
  • You must provide the Invoice Number (available on your student account information or from my.evergreen.edu - under Student Financial Services > Account Detail for Term) in the specified field.
  • Appeal must be submitted within 30 days of charge.
