Chosen First Name Request

Please provide the following information below to request to have a chosen first name (the first name you choose to use on campus in place of your legal name) added or changed on your record.  This information will be reviewed by Registration and Records staff, and if validated, will be entered into the Student Information System as your chosen first name.

A different form and process is necessary to update your legal name on record with Evergreen. Go here for more information.
